Skilled Nursing Patients Benefit from Proactive Engagement Strategies

For the last three years, Stonegate has partnered with Dallas-based Care Navigation Services to develop and operationalize proactive engagement strategies for patients recovering in one of their facilities and/or when discharged to home.

  • Through its’ Care Elevate program, Care Navigation Services staffs an on-site registered nurse [RN] Transitional Advocate who engages patients in their own self-care upon admission to the Stonegate facility, utilizing innovative clinical quality coaching strategies, coupled with post-discharge coordination.  This program guarantees patients have access to necessary resources and tools, enabling them to take a more active role in their recovery and ongoing disease management. One-on-one time between RN Transitional Advocate and patient enhances overall quality of care and patient satisfaction, while reducing unnecessary returns to an acute care hospital.

 

  • Care Navigation Services partners with a 24/7 paramedic-staffed Call Center that is customized to deliver outbound telephonic patient management via scheduled follow-ups and reminders to patients in the critical periods post-discharge to home. The program also supports round-the-clock inbound calls from patients and their family to answer non-emergent questions, thus providing a high degree of comfort during the recovery process at home.
