Meet Kim Angliss, LISW & Intake Care Coordinator
Kim has served as a Business Support and Intake Care Coordinator for seven years. Her primary responsibilities include arranging Home Health services as well as related ancillary durable medical equipment (DME) upon discharge as a patient from a StoneGate Senior Living facility.
Asked how she makes the transition for the patient of a Stonegate healthcare facility to return home, she answered: “By collaborating with a social worker to identify and align the services and equipment needed to their discharge. Doing so reduces the likelihood of admissions due to complications.” She adds that it also allows patients to continue living where they feel happiest and most comfortable — their home.”
Kim is a graduate of Texas Woman’s University.

Jennifer McKinniss | Intake Care Coordinator
Meet Jennifer McKinniss, Intake Care Coordinator
Jennifer has served as an Intake Care Coordinator for eight years. Her primary responsibilities include arranging Home Health services as well as related ancillary durable medical equipment (DME) upon discharge as a patient from a StoneGate Senior Living facility. DME’s include item such as, but not limited to beds, canes, and wheelchairs.
Jennifer says that she strives to meet residents (and caregivers) where they are emotionally and to encourage the use of Home Health services to ease the transition of returning home. “The team is committed to providing the best in transitional services — before, during, and after discharge.”
