Short Term Skilled Nursing & Rehabilitation

StoneGate provides comprehensive rehabilitation and skilled nursing recovery services following a hospitalization or acute medical decline. Whether it is physical, speech, or occupational therapy, we help get you back to health so you can back to life. Our skilled nursing facilities also offer the flexibility & convenience of inpatient & outpatient therapies.

Long Term Care

Long-Term Care at StoneGate includes a variety of services for those who cannot care for themselves. When you or your loved one can no longer be cared for in the home, we offer around-the-clock care from a specially trained team of healthcare professionals. The cheerful, home-like setting ensures the highest qualify of life.

Secured Memory Care

Memory Care provides a safe & beneficial lifestyle for those living with Alzheimer’s or other forms of dementia. Our Memory Care Units provide your loved one with a space to call home, a community to be a part of with specialized activity programs, & personalized care by our skilled & compassionate team members. Long-term or respite stays are welcome.

Assisted Living

Discover the best in Assisted Living at StoneGate. We offer a variety of programs & services designed to keep our residents active while providing the comfort & security of living in a senior community. Our two Assisted Living Facilities located in Texas offer subsidized payments through a unique partnership with the Dallas Housing Authority, meaning Texas Medicaid is a valid form of payment.

Independent Living

When you choose Independent Living Apartments at the Villages of Lakewest located in Dallas, TX, you will enjoy the freedom of being an active adult ages 55 years of age and up. Our Independent Living is subsidized through a partnership with the Dallas Housing Authority, providing qualified seniors an affordable housing solution.

Acute Inpatient Rehabilitation Hospital

Acute Rehabilitation Hospital of Plano offers short-term, inpatient rehabilitation for those recovering from a hospitalization, injury or decline in function. Patients admitting to this level of care are required to complete a minimum of 3 hours of therapies (physical, occupational, and/or speech) per day. With more intense therapies, rehabilitative nursing, & frequent physician visits, Accel Rehabilitation Hospital of Plano prepares patients to live their best life once they return home.

Outpatient Therapies

Outpatient therapies allow elderly & disabled individuals to receive physical, occupational, & speech therapies in a less restrictive setting. The patient resides in his or her home & visits the facility therapy gym 3 or more times per week. Therapy plans are customized to meet each patient’s unique medical needs.

Respite Care

Respite care provides temporary relief for primary caregivers by offering short-term care & support services to senior & disabled individuals, ensuring their well-being & safety while allowing caregivers the ability to recharge.
